IndiGo's operational failure prompts DGCA to form new 4-member panel for full review

The Directorate General of Civil Aviation has formed a four-member commitee to conduct a 'comprehensive' probe into massive disruptions in IndiGo's flight operations earlier this month. On Friday, an eight-member committee, headed by Joint Director General Sanjay K. Bramhane, submitted its probe report on IndiGo's operational chaos.
